PLANTAR FASCIITIS

EPAT treamtent for heel pain

It is typically used to treat the pain associated with heel spurs, Plantar Fasciitis, Achilles Tendonitis, Medial tibial stress reactions and other conditions. Millions of patients worldwide have found relief through EPAT and over 80 percent report less pain or no pain after EPAT treatment.

EPAT directs low frequency sound waves, also known as shock waves, at the injured area through a hand held applicator. The shock waves penetrate deep into the soft tissues, stimulating blood flow, accelerating the body’s natural healing process. Generally 3-5 treatments over four weeks are needed in our office, each lasting about 15 minutes..

Fremont Therapy & Wellness uses the FDA approved Curamedix OrthoPulse Ultra 100 EPAT(R) system for proven evidence based results.

Common Questions

What is EPAT? EPAT is a kind of shockwave therapy the delivers pulses of sound waves in a unique range to the part of your foot that is hurting. The waves  break up scar tissue and increase blood flow to areas where there is poor circulation, which stimulates healing and can also break up calcified or fibrotic tissue.

What types of conditions is EPAT used to treat? This therapy has brought relief to patients with a wide range of musculoskeletal, muscle and tendon issues. Some disorders that are successfully treated by EPAT include: Plantar fasciitis, peroneal tendonosis, shin splints, turf toe, shin splints, Achilles tendonitis, heel spurs, shoulder impingement from calcification, patellar tendonitis, lateral and medial epicondylitis and several others.

Is EPAT safe? Yes. EPAT is FDA approved and is completely safe. It has been used in Europe and other countries for many years and multiple studies have shown it to be both safe and effective. There is no danger of infection, scar tissue or other risks associated with invasive procedures.  Studies can be found HERE

Are there any side effects? There are no side effects to EPAT. In some cases, patients report minor discomfort, bruising or redness immediately following the treatment. Patients can go back to all their usual activities 24-48 hours after an EPAT treatment.

When will my pain subside?

Most patients will experience pain relief after 3 treatments, but some patients report immediate pain relief after only one treatment. Over 80% of patients report less heel pain or no heel pain after EPAT therapy. Treatment sessions take approximately 15 minutes per injured/painful area, but may vary depending on the injury. Usually three treatment sessions are necessary and are performed once every 7-10 days. If you are improving after the first three treatments but are not yet pain free, up to 5 treatments may be necessary.

How is EPAT performed?

Ultrasound gel is applied over the skin of the area to be treated. The pressure waves are then delivered using a special applicator tip which is moved over the injured area.  The machine has a loud clicking noise and patients and therapist may prefer to wear ear protection provided.

What are the possible side-effects of EPAT?

EPAT is a non-surgical treatment with virtually no risks or side effects. In some cases patients may experience some minor discomfort which may continue for a few days and/or the skin may become bruised or red after treatment.

What are my aftercare instructions?

For best results all patients receiving EPAT therapy should avoid all anti-inflammatory medication for at least two weeks prior to the procedure and four weeks afterward. This includes over the counter medication such as Ibuprofen (Advil, Motrin), Naproxen (Aleve) and aspirin. You may take Tylenol for any pain.  Re-starting the acute inflammatory process through sound waves is part of the healing and regeneration process and anti-inflammatories hinder this desired effect.

Who should not have EPAT?

EPAT should not be used in patients who have deep venous thrombosis or malignancy. It is also best to avoid the procedure if you are taking blood thinners. EPAT has never been tested on pregnant patients, so EPAT is best avoided by expectant mothers.

Am I a good candidate for EPAT?  Patients diagnosed with plantar fasciitis, achilles tendonitis and lateral or medial epicondylitis are often good candidates.  EPAT can also be used to treat several other musculoskeletal conditions successfully.  In order to find out, you will have to start with an examination by one of our Therapists. Initial Visit Physical and Occupational therapy exams are covered by Medicare and many other insurances.   In addition to a therapy exam, during the exam your Therapist will want to get a complete a brief medical history and find out about any other medical problems you may have.

Do I have to see a physician before I set up an appointment? NO – Nebraska is a Direct Access State which means that patients can seek therapy evaluation and treatment without a prescription from a physician.  EPAT can be administered by a Physical or Occupational Therapist as a form of treatment.  For traditional Physical and Occupational Therapy treatment coverage many insurances require a prescription or signed plan of care for coverage.

Will my insurance pay for EPAT?

Unfortunately, insurance companies do not pay for EPAT, though the cost of EPAT can often be reimbursed from a qualified health savings account.  Due to the cost of high deductibles, EPAT treatment is often more effective, more time efficient and can cost less than traditional physical therapy depending on insurance.  It is a much more cost efficient treatment than the downtime and effects of surgery.  It is a recommended treatment prior to any surgical intervention.
